License for medical Practice

The cost of obtaining a license to practice medicine includes:

Medical practice is an entrepreneurial activity providing services to the population in the field of improving health, diagnostics, treatment and prevention of diseases, which is carried out with a license. Business for the provision of medical services is subject to the Law of Ukraine "On licensing certain types of economic activities".

For carrying out medical practice without a license, administrative and criminal liability is provided.

Licensing is carried out by the Ministry of Health.

The license is issued for a period of 5 years.

Medical practice is carried out by business entities on the basis of a license and subject to: staffing with workers (doctors, pharmacists, junior specialists with medical and pharmaceutical education); the presence of premises that meet the established sanitary norms and rules; availability of devices, equipment, equipment

The procedure for obtaining a license for medical practice consists of the following stages:

• development of documentation based on the data provided by the client (obtaining consent for the premises for carrying out activities from the SES, fire department);
• advising the client on the proper observance of licensing conditions;
• passing the metrological verification of medical measuring equipment;
• submission of documents to the licensing commission of the Ministry of Health;
• obtaining a certificate.

To obtain a license for medical practice, the following documents are required:

• certificate of state registration of a business entity;
• a copy of the certificate of entry into the Unified State Register of Enterprises and Organizations of Ukraine;
• the charter of the enterprise (the corresponding type of activity "Med practice" must be included);
• the conclusion of the institution of the state sanitary and epidemiological service;
• certificate of the state of the material and technical base (of the established form);
• Bank details;
• VAT payer certificate number, individual taxpayer number.

 You also need documents confirming staffing, qualifications, experience, education of the doctor (s) with a category, namely:

• information on the composition of specialists (their list, full name, total work experience in this form, certification, name, graduation date);
• medical diploma; work record book with the corresponding length of service in the relevant position;
• certificate of completion of professional development;
• medical specialist certificate.

After registration you will receive: a license for medical practice.
